Description of this exam This exam is taken in a highly complex care situation.

You independently support the patient in personal care, such as eating and drinking, using the toilet and mobility and you monitor vital functions. Adapt to the capabilities and experiences of the patient. You observe and identify changes, interpret the information obtained and report this in the nursing file. You consult experts if required, make agreements about the policy used and carry out or initiate this. You are responsible for the patient’s safety. Collect information for the evaluation and discuss it with those involved. Enter modifications in the nursing care plan after

consultation.

Periodically, and if possible by the end of the health care delivery procedure, evaluate the effects of the health care delivery on the patient’s state of health and well-being.

In situations in which it occurs, write a (final) evaluation for the dismissal or transfer to another ward/care setting. Keep the nursing file up to date.

Assignment For this assignment, select a client with multiple issues in the area of self-help skills in the psychosocial and/or social area. Explore the issues of this client using theory. Involve:  the nature and causes of this client’s disorder or disability  the consequences of this disorder or disability for the way of living and the experience of the client in maintaining autonomy  the suitable supervision you will provide. Write a supervision plan in which you explain the following topics:  the component or components of personal care or psychosocial and social functioning to which your supervision is directed  the objective of the supervision  the manner in which you take into account the physical and mental well-being of the client  how you maintain and/or develop the client’s functional autonomy through this supervision  how you take into account the coping ability of the caregivers and next of kin  the specific advice you provide to reinforce the coping ability of the caregivers and next of kin.

Supervise the client according to the supervision plan you have formulated. Evaluate your supervision with the client, caregivers and next of kin.

Result The client’s self-help skills have been encouraged where possible, as a result of which he has an optimal degree of functional autonomy. You have supported the client and next of kin in a suitable manner.

Description of this exam This exam is taken in a highly complex care situation.

The nurse monitors, supervises and encourages the patient in maintaining psychosocial and social self-help skills. You encourage the client to direct his/her life and to maintain his/her identity and fulfilment of life as much as possible. You also counsel the client in dealing with and handling the consequences brought about by the handicap, disability, therapy or treatment. Provide guidance in psychosocial and social function, in maintaining and reinforcing the social network and entering into and maintaining contacts. Create the most optimal living and home environment possible for a group and include the next of kin and/or volunteers as required. Promote the most optimal group climate possible. Observe the group processes and behaviour of the clients. Help the client to obtain insight into his/her behaviour and the effect of this behaviour on others in the group. Mediate or intervene in the case of conflicts. Discuss this in the group if required. The objective is that the group of clients feels they are being treated appropriately, as per their personal needs and requirements and the client retains an optimal extent of autonomy.

Work process 1.5 Supervises a group of clients

Assignment Select a situation that affects/disrupts the group climate, or which causes the climate to be under pressure. Create a plan of action to normalise the group climate. The interventions chosen are determined by the situation, care setting and target group, and the capabilities and competency of the clients. Work according to

the regulations and instructions of the institution. Provide a clarification on your plan of action. Involve:  the situation  the group dynamics  substantiation of your choice of interventions 

the involvement of the physical aspects and mental well-being of the clients  the provision of an insight into the behaviour of the clients and the effects on the group  the manner in which the caregiver(s), next of kin and/or volunteer(s) were involved in the creation of an optimum social climate.

Description of this exam Your exam is taken in a highly complex care situation.

The nurse carries out nursing technical procedures in a competent and professional manner in accordance with the applicable protocols, laws and regulations and the guidelines of the organisation. The nursing technical procedures also include reserved and high-risk procedures. The patient with highly complex care issues is supported and means and resources are deployed efficiently in the execution of the nursing technical procedures.

Work process 1.1 Formulates the nursing diagnosis and the nursing care plan

Assignment 1 Formulate a nursing care plan for a patient with highly complex issues. Formulate the plan according to the institution guidelines, and state the objectives, activities and evaluation questions. Discuss the plan with your colleagues and the patient and his next of kin or representatives and adjust the plan based on this information as required. Insert a report of this discussion in your supporting documents. Assignment 2 Create an accountability report. Substantiate the choices you have made in the consultation. Make use of the theory concerning options, disorders, disabilities and clinical pictures. Take into account the individual situation of the patient and his next of kin. Submit a case for this patient in order to support your substantiation and state:  the course of life  the state of health  the options, disorder, handicap and/or disability and, if applicable, the clinical picture  the patient’s social network  the nursing diagnosis. Have this case signed by your work supervisor for authenticity. Result As a nurse in mental health care, you have formulated the nursing care plan based on anamnesis information and the nursing diagnosis, and discussed it with the patient and those involved. You have obtained assent.

Work process 1.3 Carries out nursing technical procedures

Assignment 1 You carry out the nursing technical procedures and reserved nursing technical procedures (see appendix) according to protocol and existing legislation and you assist the patient as part of this. Make sure every one of your procedures are evaluated. Use a training checklist to keep everything updated, the procedures you master and for which you are thus competent. If you can prove that (certain) nursing technical skills are not or rarely carried out in practical vocational training, execute these in a simulated setting. Have yourself evaluated using a random check to assess whether you have a thorough command of the work process. The following condition applies: you must demonstrate using the checklist that you are competent. Make agreements with your assessor.

Assignment 2 Write a report about the nursing technical and reserved procedure(s) you have carried out.

This report elaborates: 

the clinical picture of the patient

 the necessity of the nursing technical action(s) with regard to the clinical picture  the relevant laws concerning the nursing technical and/or reserved procedure(s) that has (have) been carried out  the manner in which you have safely and responsibly carried out the procedures with the patient. Work process 1.6 Provides information, advice and instruction

Assignment 1 Within the execution of your tasks, you provide patients with highly complex issues, next of kin and/or caregivers with clear and understandable information, advice and instructions. You provide information and advice for the benefit of maintaining or increasing self-reliance, focused on the options for dealing with an illness or impairments. Focus on the options and impairments of the patient. Create a plan on how you will provide the information or advice. Provide the information according to the plan. Assignment 2 Create an instruction plan for a group of patients, caregivers and/or next of kin. (A group can also be taken to mean a family or relatives.) The instruction may deal with:  an instruction about teaching a nursing technical procedure  an instruction about the use of an aid  or an instruction of the examinee’s choice.

Instruct a group of patients, caregivers and/or next of kin in accordance with the formulated instruction plan. Submit this instruction plan with a substantiation of how you provided the instruction.
