Please note: if you choose to provide a Statutory Declaration Form it must include:

1. the total hours of your vocational case management experience - this must be equal to or greater than the hours as stated within this Pathway (i.e. >3648 hrs) 2. your vocational case management experience in chronological order commencing with your most recent position including: the dates you commenced and ceased employment in each position (in month and year format MM /YY), your position title, your employers name (company name) and location (suburb, city/town, state). ² Vocational case management experience must be recent. An applicant must demonstrate they have practised for at least 12 months (1824 hrs) in the last 3 years. ³ Referees must have been in professional working relationship with the applicant for at least 6 months in the last 3 years and familiar with the applicant’s professional practice and their certification application.

IMPORTANT ~ PLEASE READ:

(i) All information must be typed into each CCM form (not handwritten).

(ii) Each CCM form must be printed off and signed by the individual completing the form.

(ii) Each CCM form must be certified by a JP.

(iv) Each CCM form must be scanned into a PDF file.

Obtaining a “certified copy” requires you to (i) make a photocopy of the original form or document - this is the copy that is to be certified as “a copy of the original document” by a JP (ii) Provide the JP with the original form or document and a photocopy of the original form or document (iii) ask the JP to certify the photocopy “as a copy of the original document”. CERTIFIED COPIES OF ORIGINAL DOCUMENTS Part A The following forms must be completed and certified copies submitted to the CMSA as part of your online application. The forms you are required to submit is dependent upon the vocational pathway you have nominated in your application for certification. These forms are available online at www.cmsa.org.au on the Certification page of the website. Please download and save a copy to your personal computer. Please note all information must be typed into the form, saved and then printed prior to being signed and certified. You must then scan the certified copy and save a copy to your personal computer in readiness to attach to your online application. Hand written copies will not be accepted by the CMSA. Please refer to the list below for details of the forms you are required to submit using Pathway 1, Pathway 2, Pathway 3 or Pathway 4.

1. Employment Verification Form or Statutory Declaration Form This is a compulsory requirement for all applications via Pathway 1, 2, 3 & 4 2. Professional Training and Development (PTD) Form A (Individual PTD activity) This is a compulsory requirement for all applications via Pathway 3 & 4 3. Professional Training and Development (PTD) Form B (Summary of PTD activity) This is a compulsory requirement for all applications via Pathway 3 & 4 4. Referee form 1 This is a compulsory requirement for all applications via Pathway 1, 2, 3 & 4 5. Referee form 2 This is a compulsory requirement for all applications via Pathway 1, 2, 3 & 4

¹A Statutory Declaration form may be used in lieu of the Employment Verification Form should an applicant be self-employed and/ or need to reference multiple employers/appointments for the FTE (hrs) of vocational case management experience pursuant to the nominated pathway. Please refer to STATUTORY DECLARATIONS for further information.
