Pension Contribution Waiver Application

Please note that:

• I confirm that my total ‘Adjusted Income’ for TAA purposes results in me being adversely impacted by the TAA. As a result, I wish to apply for the Pension Contribution Waiver (PCW) from the next payroll period onwards. • I understand that I will receive the standard employer Pension contribution entitlement from Fidante (I accept that I will receive no more than £10,000 in employer Pension contributions – payable over the course of a tax year). • I understand that zero routine personal Pension contributions will be payable by me (I understand that if I do have any spare Pension allowance after tapering, it is up to me to pay voluntary personal contributions – Fidante will not calculate this for me).

Important Notes

• I understand that this instruction will remain in place unless I instruct a change at a future March review point (I note that changes cannot be made at any other times).

• I understand that Fidante reserves the right to amend or terminate the PCW at any time.

• I confirm that it is my personal decision to request the PCW – I have received no advice whatsoever from Fidante.
