Lewis Drugs, Inc 401(k) Plan Required Annual Notices for Plan Year beginning 2019 Qualified Default Investment Alternative (QDIA) Notice
This annual notice advises you of certain rights and obligation you have under your Retirement Plan. This notice only applies to those who have not made an alternative election. Qualified Default Investment Alternative You have the right to direct the investment/default investments of your account in any of the investment choices offered. This includes elective deferrals and also matching and non-elective (company contribution accounts) in any of the investment choices available. If you no do choose an investment election for your contributions in the retirement plan, your account will automatically be invested in a Target Date Fund based on your age, as described below.

Right to Alternative Investment Even if the plan trustee invests some or all of your contributions in the default investment, you have the continuing right to direct the investment of your contributions in one or more of the other investment choices available to you anytime. Fees and Expenses Total Annual Fund Operating Expenses are a combination of each of the underlying investments that make up the Target Date Fund. The weighted average annual operating expense of the Target Date Funds are listed in the table above. Risk and Return Characteristics The risk associated with each Target Date Fund will be commensurate with the higher risk associated with stock investing. While moderate risk is expected, each portfolio can experience periods of extreme volatility during stock market downturns.
