Please note that all business transactions may be conducted by electronic means, including the use of electronic signa- tures and are subject to Wisconsin Statutes Chapter 137. Exclusive Distributor of… OPTIONS Multi-Dealer Inventory Management System allows you to or- der your entire practice’s inventory, dental products to office supplies, from several different suppliers. To service your unique needs, we have dedicated, professionally trained personnel to answer your questions and assist you in the supply procure- ment process. Payment… For your convenience, we accept Visa, MasterCard, American Express and Discover at point of sale and for online payment of invoices. We also accept Check by Phone and ACH Debit. Qualified customers may establish an open account with the completion and approval of a credit application. Payments on open accounts are due on all orders within 7 days after receipt of the product. Balances left unpaid after 30 days from invoice date will incur a 1.5% finance charge per month (18% APR). If we find it necessary to place an account into collection, fees and other costs associated with the collection will be assessed. Equipment… A deposit is required on all large equipment orders over $10,000. Balance of payment is due when the equipment is installed. Please note we can- not provide long-term storage of any customer’s equipment purchases, therefore equipment must be installed within a reasonable amount of time. Pricing Stability… All prices in this catalog are current at the time of publication. While it is our policy to maintain everyday low prices, market conditions and manu- facturer price increases may require necessary adjustments without no- tice. Incorrect pricing could get published due to typographical errors. In these cases, the most current pricing provided by the manufacturer will supersede the error. Prices are exclusive of applicable sales, use, and other taxes relating to the sale. Costs associated to the purchase or deliv- ery of the product shall be paid by the buyer.

All equipment orders are non-returnable and cannot be cancelled. We honor manufacturer warranties and provide our own 90-day labor warran- ty. Please read and return all warranty information required immediately upon taking delivery of your new equipment. Open or defective equipment will be covered by manufacturer warranty. Take ad- vantage of our annual preventive maintenance programs designed to keep your practice running at peak efficiency. DHP offers new equipment installation as well as old equipment teardown and disposal. All service is backed by DHP and complete warranties. Our dispatchers are available during normal business hours. Shipping & Handling… All orders within the continental U.S. are subject to a handling charge. A fuel surcharge may apply to orders. Bulk items and liquids may incur ad- ditional freight charges. Expedited freight is available at an additional cost. Hazardous Materials… The hazardous materials symbol is used to identify products classified as hazardous by the Department of Transportation (DOT). All hazardous ma- terials are subject to a hazardous materials surcharge. This charge will be added to your invoice or credit card transaction. DOT regulations prohibit the shipment of hazardous materials by air freight. Proposition 65… Proposition 65 requires businesses to provide warnings to Californians about significant exposures to chemicals that cause cancer, birth defects or other reproductive harm. These chemicals can be in the products that Californians purchase, in their homes or workplaces, or that are released into the environment. By requiring that this information be provided, Prop- osition 65 enables Californians to make informed decisions about their exposures to these chemicals. Within our web store, you will find these items clearly marked with the warning symbol. DHP updates this data upon receipt by manufacturers for their products. • Prior to returning ANY item you must call our Customer Service Department, 800.626.2163, within 30 days of the invoice date for a return authorization number. All returns must have prior consent for return by DHP which may be withheld by DHP for any reason. • Include a copy of the original invoice and the reason for return with the item. • Items may be returned if unmarked, unopened, and in original sealed packaging. • Resalable items returned 30 days after the invoice date will be subject to restocking fees. • Its is the customer’s responsibility to schedule and pay for the shipping of items returned.
