CLINICAL EXAMINATION VII. General Examination • The patient is conscious and cooperative during examination • The patient has a normal gait and posture • No organomegaly. No signs of icterus, pallor, clubbing and cyanosis were noted • Pulse: 72 beats/min • Respiratory rate: 17 cycles/min • The patient was afebrile • Blood pressure: 122/82 mmHg

VIII. Extraoral Examination No abnormality detected

IX. Intraoral Examination Soft Tissue Inspection: Two well-defined swellings are seen on the floor of the mouth on the right and left sides of the lingual frenum (Fig. 1). Swelling on the left side of the tongue is observed with respect to 35 and 36 regions, measuring approxi- mately 5 cm anteroposteriorly and 3 cm mediolaterally, extending from the lingual frenum to the mesial aspect of 36. The surface of the swelling is blue in colour and no surface ulceration is present. Swelling on the right side of the lingual frenum measured approximately 1 cm in maximum dimension mediolaterally with a bluish hue on the overlying mucosa.

Palpation: The extent of the lesion is confirmed. The swelling is non-tender. The swelling is soft in consistency, slightly fluctuant and slightly compressible. There is no blanching on pressure and no discharge is evident.

X. Provisional Diagnosis A large ranula in the floor of the mouth on the left side of the tongue with respect to 35 and 36 and a small ranula on the right side.
