2. your vocational case management experience in chronological order commencing with your most recent position including: the dates you commenced and ceased employment in each position (in month and year format MM /YY), your position title, your employers name (company name) and location (suburb, city/town, state). ² Vocational case management experience must be recent. An applicant must demonstrate they have practised for at least 12 months (1824 hrs) in the last 3 years. ³ Referees must have been in professional working relationship with the applicant for at least 6 months in the last 3 years and familiar with the applicant’s professional practice and their certification application.

IMPORTANT ~ PLEASE READ:

(i) All information must be typed into each CCM form (not handwritten).

(II) Each CCM form must be printed off and signed by the individual completing the form.

(ii) Each CCM form must be certified by a Justice of the Peace (JP).

(iv) Each CCM form must be scanned into a PDF file.

Obtaining a “certified copy” requires you to (i) make a photocopy of the original form or document - this is the copy that is to be certified as “a copy of the original document” by a JP (ii) Provide the JP with the original form or document and a photocopy of the original form or document (iii) ask the JP to certify the photocopy “as a copy of the original document”.

CERTIFIED CASE MANAGER (CCM) FORMS The following Certified Case Manager (CCM) forms are submitted to the CMSA as evidence (proof) of an applicant’s stated level of skills, knowledge, experience and/or qualification; pursuant to the nominated application pathway i.e. Certified Practising Case Manager™ (Pathway 1, 2 or 3) or Certified Case Manager (Non Practising)™ (Pathway 4).

• Employment Verification Form or Statutory Declaration Form • Referee Form 1 • Referee Form 2 • Professional Training and Development (PTD) Form A • Professional Training and Development (PTD) Form B

The CCM forms support an individual’s application and are essential for the CMSA to authenticate an application and determine if an applicant meets all of the pre-requisites for National Certification.

Important: The CCM forms will vary depending on an individuals nominated application pathway i.e. Application Pathway 1, 2, 3 or 4 and may be downloaded from the CMSA website ABOUT PROFESSIONAL TRAINING AND DEVELOPMENT (PTD) INTRODUCTION Consumers of case management services have the right to expect that Certified Case Managers™will provide services in a professional, competent, ethical and effective manner in accordance with the nationally recognised best practice standards, as described within the National Standards of Practice for Case Management (3rd Revised Edition, 2013) and National Code of Ethics for Case Management (2nd Edition, 2013). Continuing professional development is an interactive process to maintain and extend a case management practitioner or professional’s knowledge, expertise and competence throughout their career and is fundamental in the provision of safe and effective services. The CMSA defines Professional Training and Development as the movement along the continuum of competence required by case management practitioners and professionals, to provide services based upon evidence of effectiveness and best practice. This can be achieved through formal or informal instruction and learning. Active engagement in Professional Training and Development (PTD) activities is a means by which members of the CMSA, and the case management profession, maintain, improve and broaden their knowledge, expertise and competence as a means to develop the personal and professional qualities required for the specialist practice of case management.
