6.7 HIPAA A. Portability - If an employee, retiree or a covered dependent loses coverage under one of the City’s health or dental plans, the participant will receive, by first class mail at the last known address, a certificate of coverage showing how long the participant was covered by the City’s health and/or dental plan. United Healthcare will furnish the certificate of coverage for the health plan. ACS will furnish the certificate of coverage for the dental plan. These certificates may be used to reduce any waiting periods for pre-existing conditions on another group insurance plan as long as there is no break in coverage of more than 63 days. Upon request, the City People & Culture (P&C) Department will also provide a certificate of coverage for up to two years after the loss of coverage. The request should be directed to the P&C Benefits staff for active employees; retirees may contact the P&C Benefits staff for retired employees. B. Privacy - Employees who want additional information beyond what is contained in the HIPAA Notification of Privacy Practices may make an appointment with the P&C Department to review the HIPAA Procedures Manual. New employees are notified of the HIPAA rights during Benefits Sign-up.
