Evidence of the hospitals continued attempts to perform primary source verification after 72 hours and the reason why will be documented daily. PROCEDURE

Volunteer Practitioner

Completes and signs the Kern Medical’s Temporary Emergency Privileges Request Form (attachment A), which includes a statement that the information provided to the medical center is accurate. Provide proof of identification and licensure/ certification. On a case-by-case basis, at his/her discretion, grant emergency privileges or status upon presentation of any one of the following, as deemed appropriate: • Current picture ID from health care organization that clearly identifies professional designation. • Current license to practice • Primary source verification of licensure • Identification indicating that the individual is a member of a recognized state or federal response or group • Identification indicating that the individual has been granted authority by a government entity to provide patient care, treatment, or services in disaster circumstances. Assign a temporary Kern Medical identification badge signifying a volunteer practitioner status and respective licensure/certification level (e.g. RN, RT, MA, etc.). Badge will be collected immediately following the resolution of the disaster / emergency.
