December 7, 2017 Benefit Reminders DEADLINE DATES: • Changing Health Plans or Type of Coverage: December 29 , 2017 . • Flexible Spending: December 29, 2017 , return the signed form to decline or participate. • Advantica VISION: December 29 , 2017 , to elect or cancel coverage. • 401(k): You can change your contribution percentage or elect to participate at any time! • Deductible Reimbursements: April 30, 2018 , for services rendered in 2017. • Flexible Spending Reimbursements: April 30, 2018 , for services rendered in 2017 as well as health services rendered between 1/1/18 - 3/15/18 that can be applied to a 2017 Flexible Spending balance.
