14. KEY MESSAGES

• ICBT is a useful tool in the salvage treatment of superficial T1-T2 NPC recurrences.

• ICBT boost may be considered in the salvage treatment of superficial T1-T2 NPC persistent disease, or as a tool for dose- escalation in T2-T3 NPC treated with RT alone due to contraindication to concurrent chemotherapy.

• Image-guidance and 3D planning allow for better optimization and may improve control outcomes and toxicity rates.

• Twice-daily fractionation using 3-4 Gy fraction sizes are recommended.

• For BT after EBRT either as planned or salvage boost after 66-70Gy, 12-14Gy given in 4 fractions is feasible, delivering cumulative doses of 79-86Gy 10 . • For BT monotherapy for recurrent disease, 50-56Gy may be given in 10-16 fractions, delivering 62-66Gy 10 .
