VOLUNTARY SHORT-TERM DISABILITY
Short-Term Disability is intended to protect your income for a short duration in case you become ill or injured. After a 7
day waiting period for an illness or injury, you are eligible to receive 60% of your weekly income to a maximum of $1,500
per week through Mutual of Omaha. The maximum benefit period is
25 weeks. (The weekly
income benefit is subject
to a 3/6 pre-existing
condition limitation.) The
rate for this benefit is
based on your age.
