If a threat (deliberate or accidental) occurs it can
lead to:
– Mal-operation of a control system leading to
an unsafe state- an initiating event
– Mal-operation of a safety system such that it
does not operate- protection layers fail
– Both can occur at the same time
– Common cause failure

Both the above will lead to:
– Increased risk of a MAH
– An actual incident
