Please find enclosed your 2017 annual enrollment benefits booklet with an overview of the benefit plans effective
4/1/2017 – 3/31/2018. We are pleased to provide you and your family with a comprehensive benefits package
that protects your personal health and financial well being. This communication highlights changes to your
benefit plans effective 4/1/2017 - 3/31/2018.
Effective April 1, 2017 we will remain with CIGNA as our health insurance carrier with the two plan options that you
are currently familiar with. This allows you to continue to see the same providers with no concerns about switching
to a new network.
There is a minimal increase on both plan options for the 2017 plan year. Although benefit plan costs will continue
to increase, HTG continues to offer employees a market competitive and affordable benefits package. We will
continue to contribute the majority of the cost of coverage for employee and dependents on both health plan
options.
If you are currently enrolled in the Base or Buy Up plan, you will automatically continue coverage under the same
plan design with no need to complete a new enrollment form. If you are a new employee, newly enrolling in
the health plan, changing your enrollment status by adding or removing dependents or want to change plan
designs between the two plan options, you must complete a new enrollment form.
There will be no premium increase and no benefit changes to the MetLife Dental and Davis Vision plans. HTG
will continue to provide employer paid Life/AD&D and Long Term Disability (LTD). In addition, we will continue to
offer voluntary Life/AD&D, Short Term Disability (STD). This year we will be changing the carrier for the Life/AD&D,
LTD & STD plans to Mutual of Omaha. Supplemental hospital, accident and cancer plans will continue to be
offered on a voluntary basis through AFLAC.
If you will be making any changes you must complete the appropriate enrollment/change form for each carrier.
All open enrollment changes are due to Human Resources by March 24, 2017. All changes are effective April 1,
2017.
We encourage you to review the benefit plan 2017 benefits guide carefully to ensure you understand all of your
benefit choices, plans and premiums.
