PREMIUM SAVER DEDUCTIBLE REIMBURSEMENT (HRA) PLAN
WITH MORGAN WHITEMEDICAL INSURANCE
Comprehensive Behavioral Health has a Health Reimbursement Arrangement (HRA) through Morgan White Administrators. This
is available to all employees enrolled in the United Healthcare medical plan. This additional medical plan works in conjunction
with your United Healthcare plan to provide comprehensive medical coverage at an affordable cost!
Morgan White will pay
$2,500 of the employee deductible and $5,000 of the family deductible.
HOW DOES IT WORK?
Step 1:
- Present
BOTH
medical identification cards to your provider at the time of service.
Step 2:
- Claims are received by United Healthcare directly from the physician, hospital or other facility.
- The network discount is applied.
- The benefits are applied to the remaining amount.
- United Healthcare will send the hospital or physician an easy to read Explanation of Benefits (EOB) via mail or the web.
Step 3:
- The hospital or physician will mail or fax a copy of the United Healthcare EOB to Morgan White.
- Morgan White will reimburse the provider or hospital up to $2,500 of the in-network deductible for an individual
or $5,000 for a family.
In the mail, you will receive an EOB from both United Healthcare and Morgan White Administrators. The EOB from Morgan
White should match the final bill from your healthcare provider as far as your deductible responsibility for you or your dependents.
EXAMPLE:
Joe Smith has single medical coverage and is admitted to an in-network hospital for a procedure which costs $10,000. Joe shows
both
his United Healthcare and Morgan White Administrator’s identification cards at the time of admission. Below is an example
of how the claim will be processed:
Total Hospital Charges: $10,000
UHC Deductible:
$3,000
Less UHC Discounted Amount: $ 4,000
Less Deductible paid by Morgan White:
$2,500
UHC Allowed Charges:
$ 6,000
Deductible paid by Joe Smith:
$ 500
Plus Coinsurance (20 % Responsibility):
$ 500
Final Amount Paid by Joe Smith
$1,000
