either the High Deductible Health Plan with Health Savings Account (HDHP with HSA), the traditional HMO,

the Multi-Choice PPO, or waive coverage altogether.

Follow these easy steps to locate a doctor, hospital or health facility participating with Kaiser.

For members enrolled in the HDHP or HMO plan:
