AVAILABILITY OF SUMMARY INFORMATION
As an employee, the health benefits provided by Alliant represent a significant component of your compensation
package. They also provide important protection for you and your family in the case of illness or injury.
Alliant offers a variety of benefit plans to eligible employees. The federal healthcare reform law requires that eligible
members of an employer plan receive a Summary of Benefits and Coverage (SBC) for any medical and pharmacy plans
available. The SBC is intended to provide important plan information to individuals, such as common benefit scenarios
and definitions for frequently used terms. The SBC is intended to serve as an easy-to-read, informative summary of
benefits available under a plan. SBCs and any revisions or amendments of the plans offered by Alliant Insurance
Services are available by visiting AlliantNet or calling HR Solution Center at (888) 324-7471.
NOTICE OF AVAILABILITY OF ALTERNATIVE STANDARD FOR WELLNESS
Your health plan is committed to helping you achieve your best health. Rewards for participating in a wellness program
are available to all employees. If you think you might be unable to meet a standard for a reward under this wellness
program, you might qualify for an opportunity to earn the same reward by different means. Contact HR Solution Center
at (888) 324-7471 to assist you (and if you wish, with your doctor) to find a wellness program with the same reward
that is right for you in light of your health status.
