the dental manufacturers to provide you with current product information
and answer to any questions you may have about a product or promotion.
Your practice is assigned to an Account Manager who handles your ac-

sary to adjust our prices without notice. We may also need to adjust
prices that are incorrect due to typographical errors. Prices are exclusive
of applicable sales, use and other taxes relating to the sale, costs associ-
ated to the purchase or delivery of the product and all such taxes shall be
paid by buyer. Pricing in this catalog supersedes all previously published

To service the unique needs of these customers, we have dedicated and
professionally trained personnel to answer your questions and assist you
in the supply procurement process. Request for bids and proposals may

Payment…
For your convenience, we accept Visa, MasterCard, American Express
and Discover at point of sale and for online payment of invoices. We also
accept Check by Phone and ACH Debit. Qualified customers may estab-
lish an open account with the completion and approval of a credit applica-
tion. Payments on open accounts are due on all orders within 7 days after
receipt of the product. Balances left unpaid after 30 days from invoice
date will incur a 1.5% finance charge per month (18% APR). Disputed
items on an invoice are the exception. If we find it necessary to place an
account in collection, fees and other costs associated with the collection

Check by Phone Has Replaced COD’s…
Our customers are enthusiastic about our Check by Phone service. Check
by Phone is faster than COD and saves you on carrier COD fees. To use
Check by Phone, contact your Account Manager and they will collect the
necessary checking account information to get you started. No COD, no
extra charges. Returned checks will be charged a bank fee.
Equipment…
A deposit is required on all large equipment orders over $10,000. Bal-
ance of payment is due when the equipment is installed. Please note
we cannot provide long-term storage of customer’s equipment purchases,
therefore equipment must be installed within a reasonable amount of time.
All equipment orders are non-returnable and cannot be cancelled. Archi-
tectural and design services are available for a fee and are refundable
with equipment purchase. We honor manufacturer warranties and provide
a 90-day labor warranty. Please read and return all warranty information
required immediately upon taking delivery of your new equipment. Open
or defective equipment will be covered by manufacturer warranty.Ask your
