Spouse/Dependent Life Insurance
Associates currently have four options for spouse/dependent life insurance. No medical underwriting is
needed to enroll in any of the spouse/dependent life plans. The four plans and their rates are enclosed
below. During open enrollment associates will be permitted to enroll or make modifications to this benefit.
