If you wish to add or change benefits, you must complete a 2017 Enrollment Change
Form. Enrollment change forms are due to the Benefits Department on or before
January 13th, 2017
.
If you do not wish to make any changes to your current elections / coverage’s then
NO ACTION IS REQUIRED. Your current benefit election will roll over to the new plan
year.
