Brachytherapy procedures are performed without the
safeguards of Record and Verify systems.

Human errors are the main cause of inadequate
brachytherapy dose delivery, although mechanical
failures occur as well. Examples are:

exchanged guide tubes;
➢
misadjusted applicators;
➢
reconstruction errors;
➢
mechanical errors.
