EXAMPLES OF QUALIFYING EVENTS:

You or your dependent lose health coverage

because of loss of eligibility

You get married, divorced, or legally separated (with

court order)

You spouse, child or other covered dependent dies

You become eligible for or lose Medicaid coverage

You have a baby or adopt a child

You become eligible for Medicare

You or your spouse take an unpaid leave of absence

Enrolling in the Plans

All newly eligible employees are required to complete an online enrollment via the CBIZ EMS enrollment

portal. All employees are provided an Employer Paid Life Insurance policy at no cost and it is required

that you assign a beneficiary to receive this free benefit.

What you need to do…

Review your benefit materials. Use this Employee Benefit Guide to familiarize

yourself with the benefits offered to you as a POAH Communities employee.

Look at your spouse's plan or other plans you might have available to you.

Compare the costs of coverage and the benefits provided to see which

coverage makes sense for your family

.

Gather your dependent's SSN and dates of birth. All employees must have

their dependent's SSN and dates of birth if the dependent is to be covered

under any of the benefit plans

When you are ready to make your elections, log onto the CBIZ EMS portal at

www.cbizems.com

and click on "Begin Event" under the Change Events section.
