assessment!
Humanim will conduct a quarterly random prize drawing and announce the winners in October (2017) and January, April, and July (2018).

tion, exercise, managing your stress, and life/work balance. With this knowledge, you’ll have the power to make positive changes in your
life. Cigna may use your information to identify programs and services that can help you stay healthy, improve your health, or address
oth4er health-related issues. Humanim does not receive your responses to the health assessment. Humanim receives only a list of employ-
ees who have completed the health assessment which is used to draw names for the random prize drawings and Humanim receives a sepa-
rate aggregate report (summary data; no names) which is used to improve employee wellness programs.
