Formulate a maternity care plan together with the new mother and possibly her partner. Make use

of the maternity file or the maternity guide that is used in your maternity institution. In order to

guarantee the deepening of your knowledge, select situations with various care issues. For

instance:

a family with a newborn baby from a different cultural and/or spiritual background

a deviating course in giving birth or the childbed (due to an episotomy, mastitis, etc.).

a complication in the newborn child (a congenital defect, an infection of the umbilical

stump, feeding problems, etc.)

issues in the family (a brother or sister with ADHD, socially undesirable behaviour, such as

smoking near the baby, etc.).

Submit an accountability report in which you account for the choices you have made in

consultation. Use the theory and the individual situation of the family with the newborn baby.

Support your accountability by submitting a case, stating:

the course of the pregnancy

the course of the birth

observation data of the new mother and newborn baby

prevention of complications

any complications and their treatment

the care, supervision, information and advice provided.

Result

A maternity care plan with clear objectives and activities that has been discussed with the new

mother and partner. In the plan, you have taken into consideration specific requirements, options

and circumstances.

Submit the following supporting documents:

A fully completed assessment form substantiated by the examiner

A maternity care plan you have formulated and discussed with the new mother and

possibly her partner

The accountability report supported by a case
