Evaluate the support of the entire care. Evaluate with others involved. This evaluation may be a
periodical evaluation, for instance as the preparation for the Multi
-
Disciplinary Consultation (MDM)
or a final evaluation because the care has been terminated or the patient has been transferred
.
Submit an accountability report in which you account for the choices you have made
regarding the evaluation. Submit a professional product that has been signed by your supervisor
to support your report. Choose from the following options:
an evaluation report for the benefit of the MDM with the propositions to adapt the care
plan
an adapted care plan, clearly stating the adaptations carried out
a transfer with propositions for further care
You have evaluated the provision of healthcare regularly and carefully and the care plan has been
adapted as required, or you have drawn up a (final) evaluation.
Provide the following items of evidence:
A fully completed assessment form substantiated by the examiner
A report about the accountability of t
he choices made within the process of the provision
of healthcare
