57 were due to a dose less than 80% of the
prescribed dose ( underdose ),
•
35 were due to a dose to an organ or tissue out of
the treatment site that exceeded the accepted
limit. ( Overdoses of rectum, bladder wall or
prostate surrounding tissues)
