WELCOME TO VERSA-TAGS,

INC.

As a benefit eligible employee, you are offered a benefit

package which includes medical, life, long-term care, and

voluntary benefits where you can add dental, vision,

additional life, critical illness, cancer, accident, and

disability insurance for yourself, spouse, and/or children

where applicable.

Versa-Tags, Inc. offers you the choice of three medical

plans. The Base Plan has the lowest employee

contribution along with the lowest offered benefits. As

benefits increase in the Buy-Up and Enriched plans, you

will notice an increase in the contribution. Please pay

particular attention to the benefits offered under each plan

as there can be significant differences. Consider how you

and your family utilize medical care to determine which

plan is best for you.

As an eligible employee, you are also provided basic life

insurance. This protection provides a benefit of $20,000

for employees with 3 to 6 months of service; $50,000 for

employees with 6 to 12 months of service; and $100,000

for employees with 12 or more months of service. The life

benefit is provided at no cost to you.

Long-Term Care is also offered. Versa-Tags, Inc.

provides a base benefit at no cost to eligible

employees. You may elect additional coverage,

however, you will pay the additional cost. The cost is

based upon your coverage and your age at the time

you enter the plan.

Our dental and vision plans are voluntary. If you elect

these benefits, you are required to pay the full cost.

Versa-Tags, Inc. also provides the opportunity for

employees to purchase additional worksite benefits.

These benefits include Accidental, Critical Illness,

Cancer and Disability Insurance and are explained in

more detail in the following pages.

Voluntary life is an election you should consider during

your initial enrollment period. If you do not elect this

coverage when you are first eligible, you will be required

to provide evidence of insurability if you want to enroll at a

later time.

During your initial enrollment period you have the

opportunity to participate in each of the benefit plans. If

you elect to participate in the medical, dental, and vision

plans, the elections you make cannot be changed during

the benefit year (October 1, 2016 through September 30,

2017) unless you experience a major life event such as a

marriage, death, divorce, or birth/adoption of a child.
