Allegations have been made that you engaged in conduct that violates [Name of District] [policies, rules, etc.] and that could be grounds for discipline. The [Name of District] is conducting an investigation into these allegations. The purpose of the investigation is to gather the relevant facts. The [Name of District] has arranged for [Name of Investigator], [an outside investigator], to conduct this investigation. You will be given an opportunity to speak with the Investigator. Your interview is scheduled on [insert date] at [insert time] at [insert location]. You are directed to appear at that time to meet with the Investigator. You will be excused from your duties to attend the interview. You are entitled to bring a representative to the meeting with the Investigator. This representative shall not be a person subject to, or witness to, events that are the subject matter of the same investigation. It is imperative that you cooperate fully during this investigation and you are directed to answer all questions honestly and completely. [OPTIONAL: If the [District] wishes to consider giving an order that the witness keep the content of the interview confidential the [District] should obtain legal advice to evaluate whether there is an adequate factual basis to do so under the limited exceptions to the rule that blanket gag orders are unlawful. [Name of District] employees are prohibited from engaging in retaliation. You shall not retaliate against any individual who you know or who you may reasonably anticipate made a complaint, will be a witness or otherwise participate in this investigation, nor shall you attempt to influence any individual who you know or who you may reasonably anticipate made a complaint, will be a witness or other person who otherwise participates in this investigation.
