[IDENTIFY NATURE OF INVESTIGATION PER POBR, GOV. CODE § 3303(C), INCLUDING NAME OF COMPLAINANT AND SPECIFIC ALLEGATIONS, AND IDENTIFY POTENTIAL POLICIES VIOLATED, IF KNOWN, AND IF UNKNOWN AT THE TIME OF NOTICE, NOTE THAT INSTEAD.] You are ordered to report to [location] on [date] at [time] to answer questions relating to this administrative investigation. Failure to appear, or failure to answer questions, will be considered acts of insubordination and can subject you to disciplinary action, up to and including, termination of employment. Investigator [name of investigator] will conduct your interview and will be in charge of your interview. You are hereby ordered to cooperate fully with the Investigator and to answer [his or her] questions in a complete and truthful manner. You are further ordered to obey any lawful order given by [name of investigator] as though such order was coming from a superior in your chain of command within the [name of law enforcement agency]. The investigative interview will be recorded. You will have access to the recording if any further proceedings are contemplated or prior to any further interview at a subsequent time. You have the right to bring your own recording device and record any and all aspects of the investigative interview. You have the right to be represented by a representative of your choice who may be present at all times during your interview. This representative, however, shall not be a person subject to, or witness to, events that are the subject matter of the same investigation. Your representative shall not be required to disclose, nor be subject to any punitive action for refusing to disclose any information received from you while under investigation in non-criminal matters.
