Work process 1.11 Evaluates the provision of healthcare

Assignment 1 Evaluate the support of the entire care. Evaluate with others involved. This evaluation may be a periodical evaluation, for instance as the preparation for the Multi - Disciplinary Consultation (MDM) or a final evaluation because the care has been terminated or the patient has been transferred . Assignment 2 Submit an accountability report in which you account for the choices you have made regarding the evaluation. Submit a professional product that has been signed by your supervisor to support your report. Choose from the following options:  an evaluation report for the benefit of the MDM with the propositions to adapt the care plan

 an adapted care plan, clearly stating the adaptations carried out

 a transfer with propositions for further care

Result You have evaluated the provision of healthcare regularly and carefully and the care plan has been adapted as required, or you have drawn up a (final) evaluation.

Evidence Provide the following items of evidence:

 A fully completed assessment form substantiated by the examiner

 A report about the accountability of the choices made within the process of the provision of healthcare
