Contact your insurance company for requirements and eligibility. Not all plans by providers include the fitness reimbursement benefit. Once eligible, go to nihcarewards.org to register. SilverSneakers Fitness® SilverSneakers® offers free memberships to Medicare-eligible members with Humana, Medica, select Blue Cross Blue Shield and UCare health plans. Membership includes free use of the Community Center and basic fitness classes. Silver & Fit® Call the number on the back of your insurance card to check eligibility. MONTHLY MEMBERSHIP RATES Patrons with Monthly Memberships must enter a one-year agreement. The monthly fee will be deducted from either your savings or checking account on the first of each month. An enrollment fee of $30 applies to all monthly memberships.
