MSI המטופלים בשלב שני, בעזרת בדיקת ) לאי-יציבות Microsatellite Instability ( מהחולים שבהם 20%- גנומית ניתן לאתר כ הפרוגנוזה טובה. בחולים אלה אין ככל הנראה מקום למתן טיפול כימי משלים. בדיקת אונקוטייפ קולון מסייעת אף היא לצורך קבלת מאפיין נוסף של הגידול המסוים לצורך החלטה על טיפול מונע מול מעקב בלבד. האונקולוג מתחשב בהחלטתו בכל המדדים הקליניים, המעבדתיים והפתולוגיים. באשר למטופלים שאובחנו בשלב שלישי, קיים קונצנזוס באשר למתן טיפול כימי מונע המשלב פלואורופירימידינים או המקבילה קפציטבין - עם אוקסליפלטין. משך הטיפול המקובל הינו חצי שנה, אולם לאחרונה הוכח שבמקרים מסוימים ניתן להסתפק בשלושה חודשי טיפול מונע בלבד מבלי לפגוע ביעילות הטיפול.
