C. Any employee, dependent or immediate family member are eligible for the initial assessment and ten (10) free visits through the City’s Optum LAWW Employee Support Program Counselor. 6.2 Manager or Supervisory Referral A. When a supervisor or manager observes changes to employee behavior impacting the employee’s performance or attendance, the supervisor may recommend that the employee contact the Optum LAWW Employee Support Program. The supervisor should indicate the recommendation is an effort to assist the employee in becoming successful. If there is sufficient cause to believe the declining performance is related to substance abuse, the supervisor should follow the procedures in Policy H-6, Substance Abuse (page 301) . B. Whether or not the employee accepts the offer of assistance and job performance or attendance problems continue, disciplinary action procedures will apply. Discipline & Expectations (page 282) C.
