Riser de-packing

An overpressure hazard to the downstream separator is provoked if:

• The choke valve is opened too fast

• The choke not opened last – choke left open and remaining shutdown valves

opened (various combinations)

How do valves get into wrong line-up?

• Can be operator error on start-up

• Operator error on a manual shutdown

• Fail to close on a shutdown
