Typically FSAs will identify:
Lack of Competency Assessments – wrong people authoring, reviewing
and delivering FS tasks
Lack of Document Controls – No reviews, No signatures, No traceability,
No As-built
No evidence of Impact assessments for any modifications
Technical inadequacies – simply not fit for purpose
In correct purchasing decisions based on cost rather than requirements
Inadequacies in safety requirements specification – lack of involvement
from process engineering teams
