Service & Repair…
Dental Health Products, Inc.’s on-site service and repair is available for
you when you need it most. Our technical service consultants are reli-
able and manufacturer-trained. Take advantage of our annual preventive
maintenance program designed to keep your practice running at peak
efficiency. DHPI offers new equipment installation as well as old equip-
ment teardown and disposal. All service is backed by DHPI and complete
warranties. Our Service Dispatchers are available during normal business
hours. To schedule a Service Technician visit for your office, please call
your local Dental Health Products, Inc. branch office. After-hours service
is available in selected areas.
Shipping & Handling…
All orders within the continental U.S. are subject to a handling charge. A
fuel surcharge may apply to all orders. Bulk items and liquids may incur
additional freight charges. Expedited freight is available at an additional
cost. Check with your Account Manager for more information.
Hazardous Materials…
The hazardous materials symbol is used to identify products classified
as hazardous by the Department of Transportation (DOT). All hazardous
materials are subject to a hazardous materials surcharge. This charge
will be added to your invoice or credit card transaction. DOT regulations
prohibit the shipment of hazardous materials by air freight. We apologize
in advance for any inconvenience this may cause.
Returns…
Need to return an item for credit or exchange?
• Call our Customer Service Department within 30 days of the
invoice date for a RMA number.
• Please note, returns past 30 days from invoice date are subject
to a restocking charge.
• Only items listed on the RMA form will be processed for credit.
If additional items are added, a restocking fee will be applied.
• We will provide a return label and return instructions via email,
fax or postal mail.
• Enclose a copy of the original invoice, the reason for the return,
along with the item being returned.
• The customer is responsible for all freight; shipping charges will
apply on all returns.
• For your own protection, we suggest you insure the package for
the full value of the item being returned. Should the items be lost
or damaged in transit, carriers require you to file a claim directly
with them.
• Items must be returned in unmarked, unopened, and original
sealed packages making certain the item being returned is in
resalable condition.
Exceptions…
The following items are NOT returnable under any circumstance:
• Opened handpieces, small equipment, large equipment, their
accessories, and custom-ordered equipment.
NOTE: Before opening a sealed item, please examine the label to
determine you have received the correct item ordered. These items
will be replaced according to the manufacturer’s warranty as directed
by federal regulations.
• Special ordered items for products we do not ordinarily stock.
• Personalized, customized, imprinted items or government-regulated
items.
• Opened computer hardware and software.
• Hazardous items.
• Short dated or expired product.
• Discontinued merchandise.
• Items marked as non-returnable.
• Items that cannot be returned to the manufacturer.
• For X-ray film you believe to be defective, please call the
manufacturer’s help line directly. Defective X-ray film cannot be
returned to Dental Health Products, Inc.
• Certain Health-Tec
®
items such as bulk items, alloy, or imprinted
product. Please see our Health-Tec
®
Brand Products Guarantee
below for more information.
Claims for shortages or errors in shipments MUST be reported within 48
hours of receipt of delivery. NO claim for shortages will be allowed after
48 hours of receipt of delivery. We will only grant full credit on items we
can return to the manufacturer for full credit or are in resalable condition.
Re-stocking fees and other restrictions may apply. We reserve the right
to refuse any unauthorized returns following inspection. Please call our
Customer Service Department for more details.

offering you a 100% satisfaction guarantee. If you have tried a Health-Tec
®
Brand Product and it is defective or doesn’t perform up to your expecta-
tions we will provide a credit, refund or exchange. It’s your choice. Please
call our Customer Service Department toll-free, 800-626-2163, to explain
the reason for the return and to receive proper return authorization. We
have a few simple conditions that apply:
• Product(s) can only be returned within 60 days of the invoice date
with the exception of handpieces which must be returned within 30
days of the invoice date.
• Discontinued merchandise.
• Opened products must be returned in their original, unmarked
packaging.
• Hazardous, personalized, or special ordered items are not return
able under any circumstance.
• DHPI will assume responsibility for any common-carrier ground
shipping or restocking changes that may apply.
• Your account will be credited, a refund issued or an exchange made
once the return has been received, inspected and approved for final
return by DHPI.

Regulatory Requirements…
Local regulatory requirements may apply to use or installation of certain
products. Be sure to understand and comply with any such requirements
prior to purchase, use or installation of products. Any “transfers of value”
made to you by Dental Health Products, Inc. or other distributors or manu-
facturers of dental care products may need to be reported to the Center
for Medicare & Medicaid Services (CMS). This information will then be
posted by CMS on a public web site.

the Terms and Conditions at any time for any reason without notice. As a
service to our customers, we will post the most current Customer Service
Policy and Terms and Conditions on our website. The website will super-
