Trick

For each combination of treatments:

Divide mortality rates for those receiving all the

treatments by the rate ratios from the trials for

each individual treatment in turn.

Hence estimate mortality rates in the absence of

each treatment in patients judged suitable for it

Then estimate cumulative risk for patients

without that treatment.
