of applicable sales, use and other taxes relating to the sale. Costs associ-
ated to the purchase or delivery of the product shall be paid by the buyer.

Payment…
For your convenience, we accept Visa, MasterCard, American Express
and Discover at point of sale and for online payment of invoices. We also
accept Check by Phone and ACH Debit. Qualified customers may estab-
lish an open account with the completion and approval of a credit applica-
tion. Payments on open accounts are due on all orders within 7 days after
receipt of the product. Balances left unpaid after 30 days from invoice
date will incur a 1.5% finance charge per month (18% APR). Disputed
items on an invoice are the exception. If we find it necessary to place an
account in collection, fees and other costs associated with the collection
will be assessed.
Equipment…
A deposit is required on all large equipment orders over $10,000. Bal-
ance of payment is due when the equipment is installed. Please note
we cannot provide long-term storage of customer’s equipment purchases,
therefore equipment must be installed within a reasonable amount of time.
All equipment orders are non-returnable and cannot be cancelled. Archi-
tectural and design services are available for a fee and are refundable
with equipment purchase. We honor manufacturer warranties and provide
a 90-day labor warranty. Please read and return all warranty information
required immediately upon taking delivery of your new equipment. Open
or defective equipment will be covered by manufacturer warranty.Ask your

Our technical service consultants are reliable and manufacturer-trained.
Take advantage of our annual preventive maintenance program designed
to keep your practice running at peak efficiency. DHPI offers new equip-
ment installation as well as old equipment teardown and disposal. All ser-
vice is backed by DHPI and complete warranties. Our Service Dispatch-
ers are available during normal business hours. To schedule a Service
Technician visit for your office, please call 800-444-6410. After-hours ser-
vice is available in select areas.
Shipping & Handling…
All orders within the continental U.S. are subject to a handling charge. A
fuel surcharge may apply to all orders. Bulk items and liquids may incur
additional freight charges. Expedited freight is available at an additional
cost.
Hazardous Materials…
The hazardous materials symbol is used to identify products classified as
hazardous by the Department of Transportation (DOT). All hazardous ma-
terials are subject to a hazardous materials surcharge. This charge will be
added to your invoice or credit card transaction. DOT regulations prohibit
the shipment of hazardous materials by air freight.
