Diabetes Survey Deadline Extended to February 15

Ohio Revised Code Section 3313.7112 requires that all public school districts must submit

an annual diabetes survey. The online survey is

available here .

The deadline for submitting the survey has been extended to Feb. 15, 2017. The survey is

brief and should incorporate school-nurse data on diabetes for your district or school.

Ohio public school districts and chartered nonpublic schools are required to annually

report to the Department the number of students with diabetes enrolled in the district

or school during the previous school year. They also must report the number of errors

associated with the administration of diabetes medication to those students during the

same time period.
