Flexible Spending Account
The Health Care FSA and Dependent Care FSA are offered through Employee Benefits Corporation (EBC).
Please make your elections carefully as any FSA funds not used by the end of the plan year will be forfeited.
Note—if you enroll in the HDHP and contribute to a Health Savings Account, you are
ineligible
to elect the
Health Care portion of the FSA.
Rollover Reminder! Employees with balances at the end of the plan year have the opportunity to roll over up to
$500 of unused funds to the following FSA plan year. You are always encouraged to exhaust your balance
during the plan year (January 1—December 31). However, if you have money left in your Health Care FSA
account at the end of December, 2017, we will work with you, under the 2018 EBC plan, so that you can access
your unused FSA dollars (up to the $500 limit) for qualified health expenses in 2018. More information can be
found on page 12.
Employee Assistance Program
We offer an Employee Assistance Program to all employees enrolled in a Blue Cross Blue Shield medical plan
option at no cost to you. An EAP is a work-based well-being program designed to identify and confidentially
assist employees in resolving personal concerns (e.g., marital, financial or emotional difficulties; family issues;
alcohol or substance abuse) that may be adversely affecting the employee's performance or state of mind. The
benefit is available to you and your covered family members free of charge and includes up to three face-to-face
visits per incident per covered member per year. More information on the service s of the EAP can be found on
page 12.
Unum Accident and Critical Illness
Your current Accident and Critical Illness benefits through Unum will no longer be offered via payroll deduction,
effective 1/1/2018. You will have the opportunity to convert the plans to individual policies and pay the monthly
premiums directly to Unum. You will receive additional instruction from Unum regarding the conversion process
at your home address.
