Two parallel developments to reduce late effects of CSI:

evidence that with RT alone:
irradiation to limited volumes (<CSI, but
WVI/WBI) in well staged pts is equally effective
(what is the real risk of spinal seeding?)
lower RT doses (MAKEI 40 Gy --> 36 Gy --> 30
Gy) control subclinal disease
•
reports of encouraging results using a combination of
chemo followed by reduced-volumes RT
