Why did we end up with IMRT?
No technology, however fancy, will
change that.
We are therefore left with modulating
particles fluence in the cross
plane, hence IMRT.
What we were calling ‘3D conformal
RT’ was often not that conformal.
With photons, achieving dose
modulation with the falloff along the
beam direction is hopeless
