Voluntary Life Insurance - MetLife
Please note that the voluntary life coverage has a one-time open enrollment. If you have declined coverage in
the past and wish to enroll at this time, you must complete a health questionnaire and be approved for
coverage. Please contact your Human Resources Department for bi-weekly payroll deduction information.
Flexible Spending Account and Dependent Care Spending Accounts
Dependent Care and Flexible Spending Accounts work like a savings account – each pay period a pre-tax
payroll deduction is deposited into your Dependent Care or Health Care Spending Account. When you need
money to cover an eligible expense, you make a pre-tax “withdrawal” by completing a claim form or using the
Benny debit card and providing proper documentation.
Remember, what you don’t use , you lose – roll over
does not apply.

Employee Voluntary Life Insurance
You can purchase coverage in increments of
$10,000
up to a max of
$500,000
or 5 x’s annual salary.
New Hires: Guarantee issue amount of
$100,000
Spouse Voluntary Life Insurance
Increments of
$5,000
up to 50% of the employee amount to a max of
$100,000.
New Hires: Guarantee issue amount of
$25,000
Child(ren) Voluntary Life Insurance
Increments of
$1,000, $2,000, $4,000, $5000, or $10,000 max
