Voluntary Benefits Through TDS
Various voluntary benefits are also available through Tax Deferred Solutions (TDS). A welcome e-mail will be sent

UNIVERSAL LIFE INSURANCE WITH LONG TERM CARE
Universal Life Insurance with Long Term Care offers life insurance coverage through Transamerica
Life Insurance Company. The policy builds cash value with a premium that is a fixed rate for as long
as the plan is in force. Coverage is available with no medical evidence. There is also a term life
insurance option for children.
Long Term Care Rider - allows the employee to take an advance against his or her life insurance
death benefit to help pay for care. A participant diagnosed as chronically ill may receive up to 4%
of the existing death benefit if confined in a licensed Nursing or Assisted Living Facility, payable for
up to 25 months; or up to 2% of the existing death benefit if receiving Home Health Care or Adult
Day care, payable up to 50 months.
Extension of Benefits Rider with Paid up Benefit - If the entire face amount under the Long Term
Care Rider has been paid, and the insured continues to be chronically ill, this optional rider allows
an insured to have his or her benefits extended for up to an additional 25 months, not to exceed
100% of the face amount.
All of the supplemental plans are guaranteed issue for new hires, which means, there are no health
questions required to apply for coverage and be accepted. Enrollment during Open enrollment
requires Evidence of Insurability.
There are, however; elimination periods that might have to be met on some of the plans for pre-
existing medical condition. The new online enrollment system will have a PDF plan summary that
can be reviewed for each of the insurance plans being offered.
