The benefits outlined in this guide will be effective September 1

, 2017. Please take the time to review the
information and ask any questions you may have. We feel it is very important that you have a strong
understanding of your benefits. If at any time throughout the course of the year you should have questions about
how your plans work, or if you need clarification or assistance with a claim issue, please feel free to reach out to
our CBIZ consultants listed on page 28. They are always happy to assist.
Benefit Presentation
CBIZ and Humana will present your benefit options on Thursday, August 3
rd
at 10:00am. Please take advantage of
this opportunity to ask any questions you may have.
How to Enroll, Waive, and Confirm Benefits
Enrollment will be held Monday, August 7
th
from Noon – 3:30 at the High School. Everyone must complete the one-
on-one enrollment process during this time. Failure to complete your online enrollment August 7th will result in no
coverage.
Please allow approximately 30 minutes for your enrollment session. We strongly encourage you to complete the
worksheet at the end of this guide and bring it to your enrollment session. Not only will this expedite your
enrollment session, it will also ensure that you have all necessary information, including social security numbers,
to complete your enrollment.
