2016 Benefits Guide

ENROLLING IN THE PLANS

ENROLLING IN THE PLANS IS FAST AND EASY -

HERE’S HOW:

This year’s annual enrollment will be handled online through

CBAS. This system allows you to review and/or change

your benefit information from work, home, the library, or

anywhere you can access the internet, 24 hours a day,

seven days a week. When you access the online

enrollment system between November 16, 2015 through

November 30, 2015, your current elections will be

displayed. You must make any necessary changes during

this enrollment period. You are encouraged to log on to the

CBAS online enrollment website to check and confirm your

current enrollment elections or to make any changes. If

your benefits and covered dependents are not correct, you

will not be able to change your elections until the next open

enrollment unless you experience a qualifying event.

DO NOT TRY TO ENROLL PRIOR TO November 16,

2015

TO GET STARTED

ONCE YOU ARE IN THE SYSTEM



If you do not want to make any changes to your

current plan elections you do not have to log on

to the CBAS enrollment system. Your current

elections will rollover to 2016. However, we do

suggest that you log on to check your current

elections, dependents and beneficiaries for

accuracy. If you are enrolled in the Flex

Spending Account your current election will

NOT

rollover to 2016. You must log onto the CBAS

site and make your FSA election for 2016.



When you start the enrollment process, you are

asked to review your demographic information

and report any changes.



You will be directed through several screens that

will provide information on all of your benefit plan

options. Review your elections and make any

necessary corrections.



Update your beneficiary information for the Life

and Accidental Death & Dismemberment

(AD&D).



Once you have made your elections, you need to

click on the “Accept Elections” button. You will

then be prompted on the next screen to “Submit

Elections”.



After you have made your 2016 elections, print a

copy for your records.



If you make an error or want to change your

elections, you must contact Human Resources.

Changes will not be allowed after November 30,

2015.



If you experience any problems entering your

benefit elections, you may contact Asha Kuhn at

CBIZ Benefits & Insurance Services at 314-692-

2249 or Human Resources at 636-379-5438.

The online portal will be active and ready for you to enroll

from:

November 16,2015 through November 30, 2015
