Receiving Extended Coverage
If an event takes place that qualifies you or your dependents for 18 months of extended coverage, BASIC Pacific will
contact you within 14 days of receipt of a notice from you requesting extended coverage. If you die while employed,
your eligible dependents will be contacted by BASIC Pacific upon receipt of notice of your death.

Eligibility for extended coverage begins the day after the qualifying event occurs. COBRA coverage will
terminate due to any one of the following events:
•
You reach the end of your initial coverage period (18, 29, or 36 months)
•
Failure to pay in a timely manner (specified timelines would apply)
•
You become covered under another health plan without pre-existing condition limitations or exclusions
applying to you or your beneficiaries’ health plan
•
Date of Medicare entitlement
•
Advantech’s cancellation of all group plans
Once extended coverage ends for any reason, it will not be reinstated.
Detailed information about COBRA rights is given to you when you join Advantech or become eligible for health
coverage. You may request another copy of your COBRA rights notice at any time. For more information, contact
