VSP WELLVISION® PLAN
The VSP vision plan does not issue ID cards, but members can print one by
registering at . VSP members can use a VSP network pro-
vider or an out-of-network (non-participating) provider. If you use a
non-participating provider, you will be responsible for submitting a claim
form to VSP. If you use a VSP provider, the provider can confirm your enroll-
ment directly with VSP, and apply any VSP benefits or discounts at the time of
service.
When you obtain services from a VSP doctor, you get the most value from your
VSP benefit. And with the largest network of highly qualified private practice
doctors, it’s easy to find a doctor near your home or work. To verify your
doctor is a VSP doctor or to locate a VSP doctor:
• Visit or
• Call Member Services at 1-800-877-7195.
And using your VSP benefit is simple
To access your benefits, simply:
• Make an appointment with a VSP doctor
• Tell the doctor you are a VSP member when making the
appointment
• Provide the doctor with the covered member’s ID
number.
