This exam is taken in a moderately complex care situation.

In the care for the elderly, patients in rehabilitation and the chronically ill, collect information

about the patient’s health situation. Formulate a care life plan after analysing the information.

Emphasis in this plan is on ADL.

Schedule, organise and carry out the interventions chosen and agreed according to the care life

plan. When carrying out the care and support, act in compliance with the laws and regulations and

within the institution’s guidelines/prescriptions and protocols. Work in conjunction with next of

kin and informal carers. Support them and identify whether the burden does not exceed the

coping ability.

In order to provide optimum care, you identify developments in the organisation and contribute to

quality measurement and policy within the institution.
