Formulate a care life plan for a client with moderately complex issues.
Formulate this plan according to the institution guidelines. State: the objectives, the activities and
the evaluation questions.
Discuss the plan with your colleagues and the client and his next of kin or representatives and
adapt the plan based on this information as required.
Make an accountability report. Substantiate the choices you made in consultation. Use the theory
concerning options, disorders, impairments and disabilities, symptoms and the individual situation
of the client and his next of kin
.
Support your accountability by submitting a case of this client,
stating:
the course of life
the state of health
the options, disorder, handicap and/or disability and, if required, the symptoms
the client’s social network.
You have created a support plan with clear objectives and activities, which you discussed with the
client. In the plan, you have taken into consideration his specific requirements, options and
circumstances.
Submit the following supporting documents:
A fully completed assessment form substantiated by the examiner
A support plan you have co-formulated and discussed
The accountability report supported by a case
